The Veteran’s Administration Outpatient Clinic is the second largest healthcare footprint in Viera. The facility includes three primary care teams, comprised of physicians, nurses, administrative support personnel, mid-level practitioners, a social worker, dietician and pharmacists. Over 200 volunteers provide a wide variety of services to make the facility fully supportive of its patients. The facility has pioneered a program that allows eligible veterans in East Central Florida to receive inpatient medical care through a contract with other community hospitals in their area. Patients no longer have to be transported many miles from their home for care, but are treated locally.

The Wuesthoff Progressive Care Center is a 114-bed skilled nursing facility providing sub-acute and extensive rehabilitation to patients recently discharged from the hospital. The center is committed to combining traditional nursing care with strong rehabilitative and recreational therapy programs to help the patients.
The Transitional Care Unit uses a term approach to care for patients transitioning between a hospital stay and returning home. The Seniors Assistance Program provides short-term supportive services to seniors, age 60 and over. Services include housing improvement, short-term homemaking and personal care, and legal advice and assistance.
Housing Improvement services focus on environmental modifications to improve accessibility, safety, and security of the home. Background checked handymen install grab bars, build access ramps, and perform other minor home repairs. To be eligible, individuals must be 60 years of age or older and own their home. Short-Term Homemaking and Personal Care Services are provided for individuals who have experienced an illness or injury that leaves them in need of assistance for a short time while they recuperate. Home care aides may help with housekeeping duties such as vacuuming, cleaning the bathroom and doing laundry. They may also run errands, prepare light meals, and provide non-medical personal care. Home Care for the Elderly supports caring for frail elders in their home through the provision of a small financial subsidy to a caregiver to pay for such items as incontinence supplies, medications, nutritional supplements and other services to help maintain the individual at home. Individuals must be 60 or over, at risk of nursing home placement, and have income levels that qualify them for assistance under Medicaid guidelines. Case management is provided when needed.

The Shared Cost Program prevents or delays inappropriate or premature institutional placement, allowing seniors 60 and older to remain at home for as long as possible. Seniors share in the cost of services based on their income and monthly expenses increasing their accessibility to services.
The Shared Cost Program targets seniors who are unable to qualify for state funded programs due to long waiting lists or specific eligibility requirements. To qualify eligible seniors must have two limitations in their activities of daily living and have no capable caregiver.
An initial assessment, case management, and ongoing homemaking, personal care, and emergency alert response systems are the primary program services. Adult day care and in-home respite services may also be included. Senior recuperating from an acute illness participate in the program only as long as services are required. This particular help can make the difference in getting well or returning to the hospital.
An in-home assessment and financial eligibility determination are required. Seniors participating in the Shared Cost Program desperately want to remain at home. With a little help they can.

Trained, caring, and compassionate Certified Nursing Assistants and Home Care Aides provide assistance directly to seniors in their home enabling them to maintain their independence avoiding premature institutionalization. Services are designed for individuals who can handle their personal needs but may desire some assistance with everyday household tasks such as laundry, meal preparation, or light housekeeping. Caregiver respite relief is available by Certified Nursing Assistants or Home Care Aides. A companionship service helps prevent social isolation and loneliness. Home Care Aides provide the friendly visit. This service does not provide medical care.
A personalized care plan is designed to meet the senior’s schedule and needs. There is no long-term contract or commitment. All aides are members of the Aging Matters in Brevard agency and are Level II background checked and are covered by workmen’s compensation service. Call the above number for service rates. The program is licensed by the Florida Agency for Health Care Administration, license #229484 and provides homemaker and companion services throughout Brevard County.
